Who we are

National Care Providers is a registered NDIS provider based at Unit 15/38 Minchin Drive, Minchinbury NSW 2770. This policy covers the website at nationalcareproviders.com.au.

The enquiry form

The form on our contact page asks for:

• your first name and last name
• your email address
• your phone number, if you choose to give it
• the service you are interested in, if you choose one
• how your NDIS plan is managed, if you choose one
• anything you write in the goals or notes box

The page also asks whether you are a participant, a family member, a support coordinator, or a GP or clinician, and sends that along with your enquiry.

When you submit it, the enquiry is emailed to our practice inbox and it is also lodged with the client portal run by Evosion, the agency that looks after this website, so that nothing is missed. It is not published anywhere and it is not sold.

The referral form

Our referral form is longer, because it is designed for a formal NDIS referral. It asks for:

• the participant's first name, last name, phone number, email address, address and date of birth
• the participant's NDIS number, funding type, and plan start and end dates
• the plan or fund manager's name, email address and contact number
• available funds and allocated hours
• where services are to be delivered
• which services are being requested, how often, and whether an assessment is needed
• the participant's disability and any co-morbidities
• nominee or guardian details, and their relationship to the participant
• the referrer's name, phone number and email address
• how you heard about us, whether anyone else will attend, and any concerns you want to raise

A referral is saved in this website's database and emailed to our practice inbox. You are then taken to a thank you page confirming it has been sent.

What happens as you browse

This website uses Google Analytics to understand how visitors move through the site, and Google Ads to measure whether our advertising is working. Both arrive through a Google tag manager.

Between them they set a small number of cookies in your browser. They are first-party cookies, which means they are set on this website's own address rather than someone else's. The analytics ones last around thirteen months and the advertising one lasts around ninety days.

The website also loads the fonts used by the website from Google Fonts, and two animation libraries from a public code delivery network, on most pages. Loading those files means your browser connects to those services.

The website is served through a content delivery and security network, which may use cookies or similar technologies for security purposes.

There is no advertising pixel from any social network on this website, and nothing records your screen or your mouse movements. Our cookie policy explains how to control cookies in your browser.

The map on our contact page

Our contact page shows a Google Map so you can find the Minchinbury clinic. The map is loaded from Google, so opening that page connects your browser to Google's mapping service. It is the only page on this website that loads a map.
